Pennsylvania Vital Record Search

In requesting a birth certificate for any other person, including any child where one's name does not appear on the birth certificate, one must order in person or by mail. Include the following in the request: full name, date of birth, date of death, place of birth, place of death, reason for information request, name of the hospital, and signed release if necessary. Parents' names are also required, mother's maiden name for birth. The fee is $15.00 per record plus $5.50 if using a credit card. Only those parties appearing on the birth record may order via a credit card. All other parties must order by mail or in-person and give proof for the cause or reason for the request.

Access Requirements:

Records are only released to an immediate family member, a person of record, or a legal representative. A death certificate must be submitted by a family member of immediate family. Include the following in your request: full name, names of parents, mother's maiden name, date of birth, place of birth, relationship to person of record, reason for information request. Must include daytime phone number and valid government ID of requester. Records may be requested online via a vendor, but are not returned online. The internet site gives full instructions and details. The fee is $10.00 per record. There is an additional $25.00 fee for each 10 years searched, if the year is not known .

Mail search : The turnaround time 3 weeks. Credit cards not accepted for mail requests. A SASE is requested.

In person requests are honored by these cities: Erie, Harrisburg, New Castle, Philadelphia, Pittsburgh, and Scranton. Turnaround time is varies .

Records may be requested online via a vendor, but are returned by mail.
